The Project was officially launched in 2003 through extensive and detailed collaborative working with all of the Birmingham Primary Care Trusts as well as the St. Mary's Hospice and John Taylor Hospice. Since its launch it has been a huge success and supportive of the palliative community care package in Birmingham.
This service is available to terminally ill palliative care adults who are registered with a BEN PCT or HVB PCT GP in Birmingham as part of the community coordinated services to the homes of clients. Patients need to be referred by their health professional to access the service.
